Swing Bed Program

St. Martin Hospital maintains a Swing Bed Program that provides 24 hour licensed nursing care on a short term basis to create a bridge between acute care hospitalization and an alternate level of care, including discharge to home.

The Swing Bed Program is an extension of the inpatient nursing unit. Those patients admitted to the program will be screened against admission criteria for skilled care and will only be eligible for admission if it is determined that the program can meet the needs of the patient.

While the plan of care for each patient admitted to a swing bed will be individualized to meet the specific needs of the patient, each patient will be provided with:

24 hour/7 day licensed nursing care

Activity Therapy

Care under the direction of a licensed and appropriately credentialed physician

Ongoing assessment of needs and specialty referrals as deemed necessary
